That's not a marketing line — it's what the trial record actually looks like when you read it end to end. The STEP program mapped out its effects on body weight in real populations. The SELECT trial proved the cardiovascular benefit. And the compounding story that dominated headlines for two solid years has largely closed out now that the FDA-recognized shortage has ended. I want to walk through all three honestly.

I have been following the GLP-1 space since the early first-generation GLP-1 days. First-generation GLP-1 is not perfect — the side effect profile is real, the muscle preservation question matters, and second-generation GLP-1 has now been studied head-to-head against it (SURMOUNT-5, NEJM 2025). But it remains the compound with the most extensive safety and efficacy record in the class, and the only one with proven cardiovascular outcomes.

How first-generation GLP-1 works

GLP-1, or glucagon-like peptide-1, is a hormone your gut produces naturally after you eat. It does three things simultaneously: signals your brain that you are full, tells your pancreas to release insulin in response to glucose, and slows gastric emptying so nutrients are absorbed more gradually.

The natural version has a half-life of about two minutes. Your body breaks it down almost immediately through an enzyme called DPP-4. That is where first-generation GLP-1 gets clever.

First-generation GLP-1 is a modified version of natural GLP-1 with two key structural changes. An amino acid substitution at position 34 makes it resistant to DPP-4 degradation. A C-18 fatty acid chain allows it to bind to albumin in your blood, dramatically extending circulation time. The result is a half-life of approximately seven days — long enough for once-weekly dosing. Going from a two-minute molecule to a seven-day molecule while preserving receptor specificity and biological activity took decades of medicinal chemistry.

Once first-generation GLP-1 binds to GLP-1 receptors in the hypothalamus, it reduces appetite through several converging pathways. It activates POMC/CART neurons (which suppress hunger) and inhibits NPY/AgRP neurons (which stimulate hunger). It also appears to modulate the mesolimbic reward pathway, reducing the hedonic drive to eat — which is why many people on first-generation GLP-1 report not just eating less, but genuinely thinking about food less.

The gastric emptying effect extends post-meal satiety. You feel full longer after a smaller meal. Combined with central appetite suppression, the result is typically a 20-35% reduction in caloric intake without conscious restriction.

The STEP trials

The STEP clinical trial program is one of the most comprehensive datasets in obesity medicine — multiple large, randomized, double-blind, placebo-controlled trials across different populations.

STEP 1 enrolled 1,961 adults with obesity or overweight with at least one weight-related comorbidity, none with diabetes. In the STEP 1 trial, researchers studied first-generation GLP-1 at 2.4mg weekly over 68 weeks for its effect on body weight relative to placebo.

STEP 2 focused on people with type 2 diabetes and obesity. Its effect on body weight was more modest in this population, which is expected because diabetes medications and metabolic dysfunction attenuate metabolic responses. But the glycemic improvements were substantial — HbA1c dropped 1.6 percentage points.

STEP 3 added intensive behavioral therapy on top of first-generation GLP-1 and studied whether lifestyle support amplifies the pharmacological effect on body weight — it appeared to. STEP 4 asked what happens when you stop: participants switched from first-generation GLP-1 to placebo after 20 weeks regained a substantial portion of the weight they had lost over the following 48 weeks. This tells us first-generation GLP-1 is managing a chronic condition, not providing a one-time fix — consistent with the emerging understanding of obesity as a chronic neuroendocrine disorder.

STEP 5 provided the longest follow-up at 104 weeks of continuous treatment. In the STEP 5 trial, researchers observed a sustained effect on body weight at two years, with no plateau-and-bounce pattern. Participants maintained the effect for the full study duration.

Across the program, first-generation GLP-1 at the 2.4mg dose has been studied for a magnitude of effect on body weight several times larger than what any previous FDA-approved weight-management medication had shown.

The SELECT trial and cardiovascular outcomes

If the STEP trials established the effect on body weight, SELECT established something arguably more important.

SELECT enrolled over 17,600 participants aged 45 and older with established cardiovascular disease and overweight or obesity, but without diabetes. The results, published in the New England Journal of Medicine in 2023, showed a 20% relative risk reduction in the primary composite endpoint — cardiovascular death, nonfatal heart attack, or nonfatal stroke. Absolute risk reduction was 1.5 percentage points over a median follow-up of 39.8 months.

This matters for several reasons. It demonstrates cardiovascular benefits beyond the diabetes population. The 20% risk reduction is comparable to statins. And it makes a compelling case that obesity treatment itself reduces cardiovascular risk, independent of diabetes management. The mechanism is likely multifactorial — first-generation GLP-1 reduces weight, systemic inflammation, and improves lipid profiles and blood pressure, and may have direct anti-atherosclerotic effects on vessel walls. The cardiovascular benefit appears to exceed what weight change alone would predict.

Oral vs. injectable

Most people associate first-generation GLP-1 with weekly injections, but an oral formulation was also developed for diabetes. The oral version uses a permeation enhancer called SNAC that protects first-generation GLP-1 from stomach acid and promotes absorption through the gastric lining.

It works, but with caveats. Bioavailability is roughly 1%, meaning 99% of the oral dose never reaches the bloodstream. The PIONEER trial program showed meaningful results with higher nominal tablet doses, but effect sizes were generally smaller than the injectable version. Oral first-generation GLP-1 also has strict administration requirements set by the FDA label — those are part of the provider/pharmacy conversation, not something to figure out from a guide.

Injectable first-generation GLP-1 remains more potent and more predictable. The oral form is an option for people who strongly prefer not to inject, but it comes with trade-offs.

Dosing escalation

In clinical settings, first-generation GLP-1 dosing follows a gradual escalation designed to minimize gastrointestinal side effects. The specific titration schedule is set by the FDA label and managed by the prescribing physician.

For weight management, the FDA-approved target is 2.4mg weekly. For type 2 diabetes, it is 1.0mg or 2.0mg weekly. Many people receiving first-generation GLP-1 for diabetes are on lower doses than those studied for weight management.

The dose-response relationship is well characterized — higher doses have a greater effect on body weight but also more side effects. The escalation protocol exists because starting at full dose causes unacceptable nausea in a substantial percentage of people. Patience with the ramp genuinely pays off.

How first-generation GLP-1 stacks up against the rest of the class

The short version: first-generation GLP-1 is no longer the most potent compound in the GLP-1 class for its effect on body weight alone, but it remains the compound with the most extensive safety record and the only one with proven cardiovascular outcomes.

In SURMOUNT-5 (NEJM, May 2025), the first direct head-to-head trial in obesity, second-generation GLP-1 was compared head-to-head against first-generation GLP-1 over 72 weeks for its effect on body weight. That comparison confirmed years of cross-trial signal pointing the same direction. The amylin + first-generation GLP-1 combination (REDEFINE 1) and third-generation GLP-1 (Phase 2) have been studied against first-generation GLP-1 as well — though both are still investigational.

But the cardiovascular outcomes data is where first-generation GLP-1 still leads. SELECT showed a 20% relative reduction in heart attacks, strokes, and cardiovascular death in adults who had established cardiovascular disease and obesity but no diabetes. Second-generation GLP-1's SURPASS-CVOT showed it doesn't increase cardiovascular risk, but the trial wasn't set up to prove a benefit the way SELECT was. And the newer compounds — third-generation GLP-1, the amylin + first-generation GLP-1 combination — don't have cardiovascular outcomes trials read out yet at all.

If you are evaluating these compounds on their effect on body weight alone, first-generation GLP-1 no longer leads. If you are evaluating them on the totality of evidence — safety, durability, cardiovascular outcomes, real-world experience — first-generation GLP-1 still has the most complete record. What happened to the compounding landscape

The compounded first-generation GLP-1 story is essentially a two-year regulatory cycle that's now closing. Worth walking through because the cultural moment around "$129/month online GLP-1" was built on a window that has largely shut.

Here's the regulatory mechanism that made it possible. Under Section 503A of the Federal Food, Drug, and Cosmetic Act, compounding pharmacies are allowed to produce versions of drugs that the FDA has officially declared in shortage. When the FDA put first-generation GLP-1 on that list during 2022-2023, 503A and 503B compounding pharmacies started making it — often at a fraction of what the branded versions cost. Most of the compounded product wasn't quite the same molecule, though: it was a first-generation GLP-1 salt form (sodium or acetate), chemically distinct from the first-generation GLP-1 base in the approved products. The FDA spent the entire shortage saying those salt forms hadn't been shown to be equivalent to the approved drug, and there were active legal and regulatory fights the whole time over whether and how the compounding could continue.

The FDA declared the shortage resolved in 2025, and that pulled the 503A floor out from under mass compounding. Through late 2025 and into 2026 the agency actively closed the window, and reporting from Pharma Times confirms the era of widely available compounded first-generation GLP-1 is effectively over for most patients. Narrow clinical-need exceptions are still allowed — a prescriber can document specific circumstances that justify a compounded formulation — but the broad telehealth-plus-low-price ecosystem that exploded during the shortage is largely gone.

What this means practically: the millions of patients who used compounded first-generation GLP-1 during the shortage window represent a significant body of real-world experience, much of it broadly consistent with the branded product's expected effects. But that experience was accumulated under conditions without rigorous stability, sterility, or potency testing — and the FDA has issued explicit warnings about fraudulent compounded products that contain false labeling, dangerous impurities, or salt forms not authorized for compounding. The manufacturer's medical affairs position is that compounded first-generation GLP-1 is not FDA-approved and has not been reviewed for safety, effectiveness, or quality.

The compounding saga reshaped expectations about peptide access more broadly. For first-generation GLP-1 specifically, the regulatory question is largely settled. For the broader GLP-1 class — including the still-investigational compounds — the FDA has signaled that the shortage exception is not going to be a reliable long-term access route.

Side effects

First-generation GLP-1's side effect profile is real and deserves a straightforward conversation.

Gastrointestinal effects are the most common. In the STEP trials, about 44% of participants on first-generation GLP-1 experienced nausea, 30% diarrhea, 24% vomiting, and 24% constipation. These were predominantly mild to moderate and tended to decrease over time, but they led to treatment discontinuation in about 7% of participants.

The gastroparesis debate has received significant media attention. There have been reports of severe delayed gastric emptying requiring hospitalization. The mechanism is plausible — first-generation GLP-1 slows gastric emptying by design, and in susceptible individuals this effect may be excessive. The actual incidence of clinically significant gastroparesis appears low in trial data, but post-marketing surveillance is ongoing.

Muscle preservation is perhaps the most scientifically important concern. In the STEP trials, researchers observed that a meaningful share of the mass lost was lean mass, consistent with any form of caloric restriction. When body-weight changes are substantial, that share represents a meaningful absolute amount of muscle. For older adults, this raises sarcopenia concerns. The counter-argument is that proportionally, this lean mass change is no worse than diet-induced weight change or bariatric surgery, and physical function scores actually improved. A 2024 study observed that participants combining first-generation GLP-1 with structured resistance training preserved lean mass more effectively. Resistance training during treatment appears to be the answer here.

Other notable effects include gallbladder-related events at higher rates than placebo (likely from rapid weight change), a theoretical concern about thyroid C-cell tumors from animal studies that human data has not confirmed, and rare reports of pancreatitis.

The overall picture: gastrointestinal effects are common but manageable with dose escalation, gastroparesis is real but uncommon, muscle preservation is valid and should be addressed with exercise, and the risk-benefit profile is favorable for most people with obesity. This is a medication with real physiological effects, not a supplement.

Dosage research

The dosage data for first-generation GLP-1 is more thorough than for almost any peptide in the field. The 2.4mg weekly dose was selected based on Phase 2 dose-ranging studies evaluating doses from 0.05mg to 0.4mg daily. The 2.4mg dose produced the best balance of efficacy and tolerability.

The escalation schedule exists because gastrointestinal side effects are dose-dependent and adaptation occurs over time. Skipping escalation is associated with significantly higher rates of nausea and vomiting.

Some researchers have explored doses above 2.4mg. A Phase 2 study of oral first-generation GLP-1 at doses up to 50mg daily observed a greater effect on body weight at higher doses, suggesting the dose-response curve has not fully plateaued at currently approved doses. Whether higher injectable doses would produce additional benefit — and at what cost in side effects — remains an active question.

Is it safe for people without diabetes?

Yes. The STEP 1, STEP 5, and SELECT trials all enrolled participants without diabetes. Does first-generation GLP-1 cause hair loss?

For many people, first-generation GLP-1 is a meaningful pharmacological alternative, particularly for those who are not surgical candidates.

Should you combine first-generation GLP-1 with exercise?

You should. STEP 3 studied first-generation GLP-1 combined with intensive behavioral therapy and observed a greater effect on body weight than standard counseling alone. Resistance training in particular mitigates the lean mass concern that is the most commonly cited issue. The research strongly supports a combined approach.
